Health Coordination

We’re here to provide you with valuable resources to maintain your overall health, wellness and independence.

Our Registered Nurse can help with routine tasks, such as filling weekly medication boxes, administering prescribed medications/injections, providing simple wound care, assisting with diabetes management and ordering prescriptions from the pharmacy of your choice. We can arrange transportation for medical and other appointments.

We can coordinate an overall health assessment by our Registered Nurse, who can also provide education and guidance related to Alzheimer’s disease, dementia and end-of-life care.
